We recommend that for more complex enterprise applications like Salesforce.com, NetSuite, Workday, the long form review is best. It covers additional questions like your implementation approach and experience, and how you staff for management of the application.
For simpler applications like Box, the short form review is more suitable.
If you only have a high level perspective on an application, then again, the short form review is more suitable.
